Cover Picture: This article challenges the use of the Fibrosis-4 (FIB-4) index as a first-line screening tool for liver fibrosis in patients with type 2 diabetes (T2DM). Evidence suggests that FIB-4 sensitivity for detecting clinically significant liver fibrosis in patients with T2DM is only 42.6%–67.2%, highlighting the substantial risk of false-negative results and missed opportunities for timely diagnosis. The article critically examines current screening strategies and emphasizes the need to optimize early detection pathways to improve the identification of liver fibrosis in this high-risk population.
